The gift that Rose Tyler had been given isn't the simple little bit of pretty chain and beads it seems to be; and when the Dreamers wake hungry, Rose's and the Doctor's fantasies make a perfect meal. Under the Dreamers' control, they act out dark hungry passions. The aftermath is pain and hurt that the Doctor and Rose can't seem to stop inflicting on each other, themselves, and Jack Harkness. At the center of a maelstrom of emotional torment and devastation, guilt, and self-loathing is a Time Lord who blames himself for everything.
Please note that the Dreamers' domination makes their original tryst apparently unconsenting. Those two chapters are so noted and can be skipped, though issues of intention, responsibility, and guilt are subsequently addressed.
